Ireland just don't score any goals. Shane Long is comically out of form, some of his misses in the Moldova match

technically yes, the worst second placed team misses the playoffs - but I don't think that can happen in effect, there's someone in one of the other groups so shit they can't even get as many points as Ireland or Wales. Greece I think.
